Physiotherapy referrals: how to win more from doctors
Doctors stop referring when referring is hard, patients wait and nobody reports back. Five steps that earn physio clinics more referrals.
By the Kren Agency team ·
Physiotherapy referrals from doctors grow when referring is easy, the patient is booked fast and the doctor hears back. Most physio clinics lose referrals at one of those three points. Here is how a private physiotherapy clinic in Lagos or Abuja earns more of them and keeps them coming.
Why do doctors stop sending patients to a physio clinic?
A doctor who refers a patient is lending you their name. If the experience goes badly, they quietly send the next patient somewhere else. The usual reasons are ordinary:
- They don’t know what you treat. They think of you for knees and forget you for the stroke patient or the post-surgery shoulder.
- Referring takes effort. They have to find your number, write a letter and tell the patient where you are.
- The patient couldn’t get through. Nobody picked up, or nobody replied to the message.
- They never hear back. The patient disappears and the doctor has no idea what happened.

How to win more physiotherapy referrals in five steps
1. Choose the doctors whose patients need you
List the doctors near your clinic whose patients often go on to physiotherapy. Think orthopaedic surgeons, general practitioners, sports doctors, neurologists and the doctors at private hospitals nearby. Add the doctors who already send you patients, even once.
Start with ten. A short list you visit properly beats a long list you never call.
2. Visit with one clear page
Book a short visit with each doctor or their clinic manager. Bring one page that tells them:
- The conditions your physios treat, in plain words.
- Your physios’ names and what each one focuses on.
- Your address, hours and a landmark.
- How to refer, and how soon you call the patient.
Leave a small pad of referral slips with your phone number printed on them. Ask what the doctor wants to hear back, and how.

3. Make referring take one minute
The easier it is to refer, the more referrals you get. Give each doctor’s team two routes:
- The referral slip the patient brings or photographs, with the doctor’s note.
- A direct line the doctor’s team can call or message to pass on the patient’s name and number, once the patient has agreed.
Then your team calls the patient. Waiting for the patient to call you is where most referral notes die in a handbag.
4. Book the patient the same day
Call every referred patient the day the referral arrives. Offer two clear assessment times instead of asking when they are free. If they don’t pick up, send one short message and try again the next day.
Speed matters because the patient is in pain now. A week later they may have found another clinic, or given up.

5. Report back to the doctor, with consent
This is the step that keeps referrals coming. At the first visit, ask the patient whether you may update their doctor. Record the answer.
If they agree, send the doctor short updates at set points:
- After the assessment: seen, plan agreed, number of sessions.
- At a check-in: anything the doctor should know.
- At discharge: a brief summary from the physio.
Share only what the patient agreed to and what the doctor needs. The physio writes the clinical part. If the patient says no, a thank-you for the referral is enough.

How do you track physiotherapy referrals?
Keep one simple log. For every referral, write down the referring doctor, the date it came in, when you called, whether the patient booked and attended, and when you sent the update.
Review it once a month. You will see which doctors refer, which referrals never booked and where your team was slow. If a doctor who used to refer goes quiet, visit them and ask what changed.

What about gifts or payments to referring doctors?
The Medical Rehabilitation Therapists Board of Nigeria (MRTB) is the regulator and licensor of physiotherapists in Nigeria and publishes a Code of Ethics. Before you offer a referrer anything of value, check that code, and the referring doctor’s own professional rules, with someone qualified to advise you.
The referrals that last are earned with fast bookings, good care and useful updates.

Questions clinic owners ask about physiotherapy referrals
How many doctors should we start with?
Start with about ten doctors you can visit and follow up properly. Add more once the referral route and the same-day calls run smoothly for those ten.
Can we update the doctor without asking the patient?
Ask the patient first and record their answer. If they say no, thank the doctor for the referral without sharing details about the patient.
What if a referred patient never answers our call?
Call on the day, send one short message, then try once more the next day. After that, stop chasing and note it in your referral log.
Should we only rely on doctor referrals?
Referrals are valuable, but many people with back pain or a sports injury look for a physio themselves. Showing up on Google for the conditions you treat brings in those patients too.
