How much should my clinic spend on marketing each month?

A quick monthly worksheet: count your open slots, the most you can pay per booking and your fixed costs, and the right monthly marketing figure falls out.

By the Kren Agency team ·

How much to spend on marketing each month is a question every clinic owner asks, and no fixed figure fits every clinic. The right monthly number comes from three things you can count: the bookings your team can still take, the most you can pay to win one booking, and the costs that run every month whatever happens. Put those on one page and the figure works itself out.

Our post on setting a clinic marketing budget explains how to find what a new patient is worth. This post is the quick monthly version: a worksheet you can fill in with your own numbers in about fifteen minutes.

How much should a clinic spend on marketing each month?

Spend up to the point where every booking still costs less than you can afford, and no further than your team can handle. Use letters for now. You fill in the numbers from your own records.

  1. A: open slots. Count the appointments you can still take each month for the treatment you want to grow. Use the real gaps in your bookings, not the hours on the door.
  2. B: new bookings wanted. Decide how many of those slots you want filled by new patients. B can never be more than A.
  3. C: most you can pay per booking. Take this from the patient value method in the budget post. It must cover everything, ads and people included.
  4. Ceiling: B × C. This is the most your clinic should spend this month to win new patients for that treatment.
  5. D: fixed monthly costs. Add up what runs every month regardless: the people managing it, reminder messages, website upkeep, content.
  6. Ad money: ceiling minus D. What is left is what you can safely give Google or Meta this month.

If ceiling minus D comes out small or below zero, don't force ads. Fix the leaks and lean on channels that cost little to run, then rerun the sheet next month.

Six lines, your own numbers, one monthly ceiling.

What goes into the monthly number?

Owners often count only the ad bill. Then the month ends over budget and nobody knows why. List every line that recurs:

  • Ad spend: what Google or Meta charges for showing your ads.
  • People: your own marketing staff, or the team you pay to run ads, pages and follow-up.
  • Messages: reminder and follow-up SMS. Our post on bulk SMS reminders for clinics covers how these are charged.
  • Content: photos, short videos and design for posts and ads.
  • Website upkeep: hosting, domain and small changes.

Keep one-off costs out of the monthly sheet. A new website or a photo shoot is a separate decision with its own budget. Mixing them in makes a normal month look expensive and a cheap month look like a win.

Recurring lines go on the sheet. One-off costs sit outside it.

Why do your open slots set the ceiling?

Marketing creates enquiries. Your team turns them into bookings. If your bookings are already full on the days patients want, more spend brings people you can't see quickly. They wait, they cool off, and they book somewhere else.

So check A before you raise anything. If the busy days are full and the quiet days are empty, the cheaper fix may be moving patients across the week. Our booking operations work does exactly that before the ad money goes up.

Test months and steady months

A new channel needs a test month before it earns a steady budget. Start below your ad money line, run one treatment in one area, and judge it only by cost per booking. If it books patients under C, it moves to steady. If not, fix the step that broke or drop it.

You are not locked into the month. Google says you can set and edit your average daily budget at any time. Google also says one day can spend up to twice the daily figure for most campaigns, while the month stays capped. A single busy day is normal. Judge the month, not the day.

When should the monthly number change?

Rerun the worksheet in the first week of each month. Change the number only when one of the letters changes:

  • A changes: a new doctor joins, a room opens, or a treatment goes on hold.
  • C changes: prices move, or a channel keeps booking above your limit.
  • D changes: you add a person, a tool or a new content line.
  • The season changes: some months bring fewer patients. Our post on filling slow months covers what to do then.

If none of the letters moved, keep the number. Steady spend is easier to read than spend that jumps every month.

How Kren works out the number with you

Kren is a done-for-you growth partner for private clinics in Lagos and Abuja. We fill in this sheet with you, close the leaks first, then set up and run the website, Google, ads and follow-up automations. Your own team does the final follow-up and booking. See our sales strategy work for how the numbers connect.

Questions clinic owners ask about monthly marketing spend

Can a clinic market itself without paying for ads?

Yes, to a point. A complete Google Business Profile, fast replies, follow-up and recall of past patients cost little to run and often book patients. Ads add reach on top once those are working.

Should we spend the same every month?

Keep it steady unless a letter on the sheet changes. Steady spend shows you what each channel really does. Jumping up and down makes every month hard to compare.

What if we can't work out the cost per booking yet?

Start tracking now. Record where every enquiry came from and which ones booked. After a month or two you will have a real figure to use for C.

Is the ad bill the whole cost?

No. People, messages, content and website upkeep sit on top of it. Count them all, or the month will cost more than you planned.
