How to start a hospital in Nigeria: the steps, in order
The steps to open a private hospital in Lagos or Abuja, in order: business registration, premises, staff licences, equipment, facility registration and first patients.
By the Kren Agency team ·
How to start a hospital in Nigeria comes down to seven steps, in this order: decide exactly what you are opening, register the business with the Corporate Affairs Commission (CAC), secure suitable premises, hire licensed staff, buy registered equipment, register the facility with the regulator, then line up your first patients before opening day. In Lagos that regulator is HEFAMAA. In Abuja, it sits within the Federal Capital Territory Administration.
Each step below points to what the official bodies say, and to our deeper posts where we have them.
Step 1: Decide what you are opening
"Hospital" is a legal category, and the regulator holds you to it. In Lagos, HEFAMAA defines a private clinic as premises where care is given on an out-patient basis, with observation before referral to a hospital. Its registration schedule says no clinic shall have more than 2 beds. Hospitals are graded by bed count.
So settle these early:
- Clinic or hospital. Admissions push you into the hospital category.
- Services. General practice, maternity, dental, diagnostics, physiotherapy and others are listed as separate categories.
- Size. Bed count shapes premises, staffing and fees.
Running outside your registered category is a monitoring offence, so be honest about scope from day one. Put all of this into a written plan first. Our hospital business plan breakdown walks through each section.

Step 2: Register the business with CAC
The CAC handles three kinds of registration: Company Registration, Business Name and Incorporated Trustees. Applications start online from the CAC website through its registration portal.
Choose the structure with your lawyer or accountant, especially if investors are involved. Keep your CAC certificate and number to hand for the registrations that follow.
Step 3: Secure the right premises
HEFAMAA says it prefers a purpose-built building for a health facility. Where that isn't possible, it expects you to consider the safety of patients, staff and the host community, with strong infection control and waste management (HEFAMAA FAQs).
Before you sign a lease, check that the layout allows clean patient flow, safe waste handling and the rooms your services need. Walk the space with your medical director, not just the landlord.

Step 4: Hire licensed staff
You don't have to be a doctor to own a hospital in Lagos. HEFAMAA allows a non-medical owner if the operating officer is a qualified medical practitioner with updated licences who signs a bond with the agency. An undeclared change of operating officer is an offence.
Check every professional's licence yourself:
- Doctors and dentists hold practising licences from the Medical and Dental Council of Nigeria (MDCN). Our MDCN portal post covers registration and annual renewal.
- Nurses and midwives are regulated by the Nursing and Midwifery Council of Nigeria (NMCN), which offers online licence renewal and certificate verification.
- Laboratory, pharmacy and other staff answer to their own councils. Confirm each one before they start.

Step 5: Buy equipment that is registered
The National Agency for Food and Drug Administration and Control (NAFDAC) registers medical devices, including in vitro diagnostics. Its medical devices division processes applications for both imported and locally made devices, submitted through the NAPAMS portal.
Ask every supplier for proof of registration before you pay. Our hospital equipment list for a new clinic goes room by room so you buy what your scope needs, and nothing it doesn't.
Step 6: Register the facility before you open
In Lagos: HEFAMAA
Under Lagos State's Health Sector Reform Law, no one may run a health facility on premises not registered with HEFAMAA. You apply on the e-HEFAMAA portal. HEFAMAA's FAQ says there is one inspection before final registration, then monitoring visits twice a year. Running unregistered attracts a ₦100,000 fine when discovered.
Renewal is yearly, and late renewal costs extra: 50% of the renewal fee from April to June, rising to 150% by October to December. Our HEFAMAA registration walkthrough lists the documents and each portal step.
In Abuja: FCTA
The FCTA's Health and Human Services Secretariat lists regulatory and oversight functions for all private medical service providers. One of its departments is the Private Health Establishment, Monitoring & Registration Committee (PHERMC) (FCTA: Health and Human Services Secretariat). Contact the secretariat directly for current forms and fees before you commit to premises.

Step 7: Plan your first patients before opening day
A certificate lets you open. It doesn't fill a single booking. If marketing starts the week you open, rent and salaries are already running while the bookings are empty.
Start a few weeks before launch:
- Get found. Set up your Google Business Profile so people nearby see you on Google Maps.
- Give them a page that books. A clear website listing your services, location, hours and how to book.
- Answer every enquiry fast. Calls, DMs and forms need a named person and a reply time.
- Track bookings weekly. Enquiries in, bookings made, patients who showed up.

Licensed and ready. Now fill the bookings.
